Czech Birth Certificate

I want to try and get a copy of my great grandfathers birth certificate. The information I have found says he was born in Kamen. I found a form that can be filled out and sent to the Embassy of the Czech Republic, but it needs Place of birth and District. Is Kamen the place of birth? What is the District?
Any suggestions would be great.

I have found 4 Kamen locations-this would be the town name. One is located in the Caslav area in middle Bohemia, another is also in middle Bohemia Pelhrimov area, one in west Bohemia Prestice area, and one in NW Bohemia Kraslice area. You will need to find additional information to try to pinpoint which one.
